How does one..
... twist a testicle? Is 'specialised' apparatus required? Certainly not something I'm an expert at, but I'd imagine finding enough purchase on one to twist it to be quite tricky. Squeeze, prod, or otherwise applying a force, yes, absolutely, makes sense. But a twist suggests two opposing rotational forces - quite a tricky manoeuvre, and certainly one that I would imagine impossible without the subject being either willing or incapacitated.

I've done a little research.
And frankly, I wish I hadn't. It's the sort of thing any man might lose a lot of sleep over.
http://en.wikipedia.org/wiki/Testicular_torsion
"Testicular torsion is a surgical emergency that needs immediate surgical intervention. If treated within 6 hours, there is an excellent chance of saving the testicle. Within 12 hours the rate decreases to 70%, within 24 hours is 20%, and after 24 hours the rate approaches 0. (eMedicineHealth) Once the testicle is dead it must be removed to prevent gangrenous infection."

That'll teach him to lose his man at set pieces.
